For certain immigration applicants, a medical examination is required as part of your application. Dr. Brian Madden is a certified civil surgeon of the USCIS (formerly INS) authorized to perform these examinations. You may call the office to schedule an appointment at any time for this examination. Dr. Madden speaks Spanish and there is Japanese translation also available.
Please bring the following to your visit:
- Your passport and A-file number
- Any health records or vaccination cards you may have
- Lab test results within the past 6 months, if available
Depending on the tests required, we can have your I-693 forms prepared within one week or sooner. You do not need to provide the forms. The cost of the examination is $125, plus any testing or vaccinations required, and is not covered by insurance.
