UHP (HMO)
If your health plan is with one of the
companies listed above, we accept your insurance. Please contact our
office if you have any questions or if your plan is not listed above.
We also accept Medicare and payment by credit card or cash at the time of
service. For uninsured patients, we can help you predict the costs for your
given situation and provide assistance on our own services and on most laboratory and radiology tests ordered from the
office for those who qualify.
All of our patients are also encouraged to view
our resources to find out more about their own insurance options and
benefits.
Insurance Information - HMO plans
If you have an HMO plan and are looking for a new physician, you may
contact your insurance company directly and request to have Dr. Jodie Escobedo, Dr. Brian Madden, or Dr. Wakana Saeki
assigned as your Primary Care Physician. Depending on your insurance company and plan, one or more of the Peak physicians should have an open panel for you to join.
Currently, the Peak physicians have open panels under the Bay Area Medical Group IPA and Prospect Healthsource IPA. As the IPA, not the primary care provider, determines the specialty network and affiliated hospitals, please be aware of the panel's affiliated IPA medical group if you have a particular preference for a specialist physician or hospital that should be "covered under the HMO".
Any of the physicians in the Peak Health offices are authorized to
provide services for patients assigned under another Peak Health physician.
Even if you have HMO insurance under a plan listed above, if these physicians are not
assigned to you as your primary care physician, your insurance may not
reimburse us for your visit and we may ask that you make other
arrangements for payment at the time of service. Some HMO plans will
allow for partial reimbursement for our services (i.e. tier II level of
service) even if we are not assigned
as your primary care providers - contact the office
or your insurance company for more information.
